Medellín has two airports, and this matters
Medellín has two commercial airports. Almost everyone uses one.
José María Córdova International (MDE) (aeropuertorionegro.co · Km 4 Vía Llanogrande, Rionegro) is in Rionegro, 35 km east of Medellín, at 2,150 m altitude. This is the airport for international flights, most domestic flights, and almost everyone you know. Drive time to El Poblado: approximately 45 minutes off-peak via the Túnel de Oriente, 60 to 90 minutes in traffic or via the Las Palmas highway.

MDE is a single-terminal airport with one international and one domestic concourse. The terminal is modern, with the usual amenities: cafes (Juan Valdez and Pergamino both have outposts), pharmacy, ATMs, currency exchange, free wifi (you do need to register with an email).
Arrivals layout: exit the customs area, you will see exchange counters and the official taxi/transport desks immediately. Ignore the people offering rides as you walk out, they are unofficial and often overpriced. Walk past them.
Official transport area: just outside the main exit doors. Yellow airport taxis line up on the left, official shuttle (Combuses) buses on the right. The cheapest option for independent travellers is the shared colectivo (mini-bus) that runs between MDE and central Medellín. Combuses is the main licensed operator. Fare: approximately COP 14,000 to 16,000 per seat (approximately USD 3.50 to 4.00). Departures run roughly every 30 to 60 minutes during the day, less frequent at night.
When to use: budget travellers without much luggage, solo arrivals heading to a hotel near a Combuses stop (San Diego mall area, the city centre). Not door-to-door. You finish the trip with a short taxi or walk.
Where to catch: follow signs from arrivals to the transport zone, then the "Combuses" counter immediately outside the terminal. Buy a ticket at the counter before boarding.

Private driver, the worth-it option
For families with kids, business travellers, or anyone who wants door-to-door service without app friction, a private driver is the easiest option. Pre-booked, signs with your name at arrivals, fixed price, English-speaking on request.
Typical fare: COP 80,000 to 140,000 (approximately USD 20 to 35) one way for a pre-booked driver, depending on vehicle type and operator. Prices above COP 140,000 are worth negotiating or comparing with a ride-hail quote.

Official taxis are always there. The yellow-taxi queue at MDE runs 24/7. This is your fallback.
The Las Palmas highway is well-lit and well-policed. Driving down at midnight is safe. There is no need to fear the journey itself.
Hotels. Confirm with your hotel they will be open and have someone at the desk to let you in. Most El Poblado hotels are 24-hour but smaller B&Bs are not.
Leaving the city: timing, traffic, paperwork
Departure timing: leave El Poblado at least 2 hours before an international departure, 2.5 hours during weekday rush hour (7 to 9 a.m. or 4 to 7 p.m.). The Las Palmas highway can be very slow.
Tunnel option: Túnel de Oriente (tuneloriente.com, operator Concesión Túnel Aburrá Oriente) (eastern tunnel) is a newer, faster route through the mountain. It charges a toll (COP 17,000 to 25,000 depending on time). Worth it when traffic on Las Palmas is heavy. Most app and taxi drivers know to default to it during rush hour.
Check-in: domestic flights open check-in 90 minutes before departure, close 30 minutes before. International flights: 3 hours and 60 minutes respectively.
Departure tax: for Colombian residents on international flights, there is a small tax built into the ticket. Foreign visitors generally have this included automatically. Verify with your airline.

Conductor privado, la opción que vale la pena
